Find a physician

The first step to healthy living is finding a geriatric physician and building a healthcare relationship. It is important to find a doctor close by so you can easily access care when you need it most. We're here to help you.

Even if you have no urgent care needs, your primary care physician can help with preventative medicine. You can prevent serious health conditions with annual physicals that include lab work and health screenings. They can also refer you to health services such as skilled nursing facilities and senior home health care.

Physical therapy and rehabilitation

Our physical therapy and rehabilitation unit is run by a physician-directed team of specialists who work together to help each patient reach their optimal results. Patients stay in the facility for approximately two weeks and receive three hours of therapy five or more days a week.

Palliative care

Palliative care is specialized medical care for people living with serious and chronic illnesses. Rather than treating a condition, the goal of palliative care is to improve the quality of life for both the patient and their family. Our palliative care team is devoted to working with you and your family with this transition of care. We have case managers who also offer support during this time.

Hospice care

Hospice care is for people who are nearing the end of life. A team of healthcare professionals provides comfort for a person who is terminally ill by reducing pain and addressing physical, psychological, social and spiritual needs. To help families, hospice care also provides counseling, respite care and practical support.
